Great question! Since the patient had a LBBB and the ECG met sgarbossa criteria (and she was so sick) we didn't get a posterior ECG on her. I usually worry about posterior MI when I see ST depressions in the anterior leads (like we do with this patient) but since it's a LBBB, that pattern should always hit sgarbossa criteria and prompt cath lab activation. It would have been very interesting to see what a posterior ECG would have shown us. Great question! I'll have to get a posterior ECG on the next patient who comes in with depressions like this with a LBBB and see what it shows. Thanks!
